In the final fight between the Uchiha brothers, Itachi was also nearly blind from his overusage of his Sharingan. Because of this, his illness worsened. Moreover, in their last moment, he claimed to always have loved his little brother. In fact, Itachi has completed his overall plan of protecting Sasuke and removing Orochimaru from his body. Eventually, he essentially did everything on his to do list to ensure, at least in his mind, to keep Sasuke safe.
